• If you have medical or dental insurance, please bring all the necessary information so that we can expedite reimbursement.
• prior diagnosis and treatment rendered
• Prior X-rays/splints/night guards if available.
• A list of medications you are presently taking.

Parking

Surrey Clinic: Two dedicated free parking labeled as "Parking for unit 170" is available at the commercial parking area just next (north) of the clinic off King George Blvd (See Map). Free accessible (handicapped) parking space is available near the entrance of the clinic.

Vancouver Clinic: One-hour free parking is available in front of the clinic on Kingsway from 9:30 am onwards. Free parking is also available on Inverness Street and 20th Avenue.
